Are you a Carer?
A carer is someone looking after a friend or a
relative of any age who through an illness, disability
or addiction, is unable to manage without help
Why should I identify myself as a carer at the
surgery?
So that we can help and support you.
When can I identify myself as a carer?
At any time when you are in the surgery, just tell
the doctor, nurse or health visitor, send an email to
Cropredy.surgery@nhs.net or if you prefer, contact
the local carers? centre at the address below:

Who should I contact in an emergency, say, if I
am unexpectedly admitted to hospital?
If you register with Emergency Carer Support they
will help you to manage for the first few days until
alternative carer support can be found. The service is
currently provided by A4e call on 01865/374430 or email
oxfordilsenquiries@a4e.co.uk
Who will see the surgery record?
The GPs, Nurses and practice staff who keep the
records but we may also let the hospital know if you are
admitted.
Where else will this information be used?
We may use anonymous data to help plan further
services for carers.
