If you move house, change your name or telephone
number please remember to let us know so that we can

Your Confidential Records
Your personal records are confidential. Unless you give
your written permission e.g. for a relative or carer to
have access, your personal information will not be
disclosed.
There may be occasions, however, when the law requires
us to pass on information e.g. to notify infections
diseases when they may endanger others' safety.
You have a right of access to your health records. We
are entitled to make a charge for them to cover, for
example, for the cost of identifying and locating the
information, copying and postage.
We comply with the Data Protection Act and the Freedom
of Information Act.

Comments and Complaints
Please let us know if you have a comment or suggestion
about the service that we offer. There is a suggestion
box in the waiting room and although some of the
requests are a little unusual such as the one asking for
pillows, we do read them all and try and respond where
we can.
If you are unhappy with any aspect of the service,
please speak to a member of staff as soon as possible so
that we can put things right. A leaflet outlining the
way in which we deal with complaints, is available from

If you would like to speak to someone outside
the surgery, please call the Patient Advice and Liaison
Service (PALS) on 0800 052 6088.
Rights and Responsibilities
Each of us has a right to be treated with respect and
dignity and so we are committed to doing all that we can
to protect patients, visitors and staff from
unacceptable behaviour and will not tolerate any
incident that causes hurt, alarm, damage or distress.
Any patient who is abusive or violent may be removed
from the practice list.
Privacy
Please ask if you wish to speak to the Receptionist
in private.
Doctors' Holidays
Both Doctors carry out each other's surgeries during
holiday times. Occasionally locum doctors will
stand in.
