What is Medlink and Medlink Patient FAQs

Medlink is a secure online questionnaire system used by GP practices to help you complete parts of your long-term condition (LTC) review and medication monitoring from home. It saves time and helps ensure your care is up to date.

Medlink Patient FAQs

Why have I received a Medlink message?

  • You may receive a Medlink link if:
  • · It is time for your annual long-term condition review
  • · You take medication that requires regular monitoring
  • · Your clinician needs updated information about your symptoms or treatment

Do I need a login or password?

No. Medlink does not require an account. Just tap the personalised link we send you by text, email, or the NHS App.

How do I complete the questionnaire?

  • · Tap the link in your message
  • · Answer the questions at your own pace
  • · Your progress saves automatically
  • · You can return later using the same link
  • · You may ask a family member or friend to help you

What is Medlink medication monitoring?

Some medicines require regular checks to keep you safe. Medlink helps us monitor medications such as:

  • · Methotrexate
  • · Lithium
  • · Warfarin
  • · Certain inhalers
  • · Other long-term treatments

You may be asked about side effects, how you take your medication, and whether you need blood tests or a review appointment.

Will I still need tests or appointments?

Possibly. Depending on your answers, Medlink may direct you to book:

  • · Blood tests
  • · Blood pressure checks
  • · Foot checks
  • · Other physical measurements

A clinician will review everything once your results are back.

Can I complete Medlink on my phone?

Yes. Medlink works on smartphones, tablets, and computers. No downloads or apps are required.

Who sees my answers?

Your responses are reviewed by our clinical team and added directly to your NHS medical record.

What if I can’t use the internet?

If you cannot complete the online questionnaire, please contact the practice. We will arrange your review in the usual way.

Is Medlink secure?

Yes. Medlink uses secure NHS-approved systems. Your link is unique to you and cannot be used by anyone else.

How long does it take?

Most questionnaires take 5–15 minutes, depending on your conditions.

What happens after I submit it?

A clinician reviews your answers and test results. You will be contacted if:

  • · A follow-up appointment is needed
  • · Your medication requires changes
  • · Your review is complete

Using your NHS account or Patient Access

There are other tasks you can do online, like:

  • book appointments
  • order repeat prescriptions
  • view your test results
  • see parts of your health record
  • check or cancel appointments
  • see your vaccination record

For these, you can use your NHS account or Patient Access. You can access them using their websites or apps.

Setting up an NHS account/NHS App

To set up your NHS account for the first time, you will need to set up an NHS login. Read about how to set up NHS login (nhs.uk).

Once you have set up your NHS login, you can log in to your NHS account or download the NHS App onto your phone.

Setting up Patient Access

To set up Patient Access for the first time:

  • visit the surgery with photo ID so we can confirm your identity
  • we will give you a username and password
  • go to Patient Access or use the app to register
